The Dodge County Department of Human Services has contracted with Lutheran Social Services to provide CCS facilitation. Providers would create a team of wraparound services and providers that will meet the needs of the CCS qualifying client and their family. CCS clients must pass an assessment completed by the county after being referred by an outside source. Clients are usually involved in the CCS program for a year or longer depending on need.
Family and Community Professionals work collaboratively with the family, social workers, school and other provides to ensure a cohesive approach for the youth and family. Family and Community Professionals participate in team meetings and develop treatment objectives with social worker input.
Family Preservation Services typically work with families and youth from 6 - 17 years of age in and around Dodge county.Since this program serves several counties, there is a significant amount of travel required for the role and there is likely to be daily travel based on the needs of the referrals. Family and Community Professionals are reimbursed for their mileage and can bill for travel time to and from the client's home, as well as travel with clients in the community.
The right candidate will be comfortable with an unstructured, flexible schedule, traveling in and around the tri-county area, going into homes and exposure to all associated environmental factors, including cleanliness/pets/smokers, etc., and will be knowledgeable and aware of secondary trauma and self-care to promote sustainability. There will be a lot of afternoon and evening hours as staff will primarily be working on skill development with youth. There is a good chance staff will be able to run groups as well. Some weekend hours are possible, but this will be flexible and somewhat at their discretion.
Under direction, performs work involving the delivery of professional social work services to individuals, families, and groups. Performs other duties as required.

CERTIFICATES, LICENSES, REGISTRATIONS:
The incumbent of this position must have a valid driver's license and have reliable transportation to perform the essential duties of the role; a motor vehicle check (MVR) with a satisfactory driving record per the LSS Driver Safety Procedure is required, and ability to meet LSS auto insurance requirements.
TRAVEL: Ability to travel on day trips as required up to 80%. Some overnight travel may be required.
